- This membership offers a wide range of allowances for sharing in a broad spectrum of wellness and healthcare needs. It is supported by robust resources designed to help and empower you to make informed decisions about your health.
- A pregnancy is eligible for sharing if it begins after 90 days of effective Membership and the estimated date of delivery falls one year or after the Membership becomes effective.
What's Included?
Preventive Care
This membership shares in the cost of routine general wellness care per the national recommendation, including:
- annual wellness exam
- well-woman visits and pap smears
- well-child visits from birth through age 18
- standard screenings, like mammograms (40), colonoscopies (45)
- STD/STI screenings
- birth control
- routine lab work
- routine immunizations
You’ll also receive an additional annual $200 Self-Care credit to use towards additional wellness services, like gym memberships, maintenance chiropractic care, nutritional supplement costs, etc
Primary Care & Specialists
This membership does not restrict you to networks. If you wish to keep your provider, simply ask for the “self-pay” or “cash pay” rate to apply any applicable discounts or ask the Member Care Team to locate a self-pay-friendly provider near you. If a new medical condition or medical Need develops, your payments will accumulate towards your chosen IUA before being eligible to share
Virtual Care
Your virtual visits for acute medical concerns will be reimbursed up to $89 at the provider of your choice. Consider this virtual provider platform though indipop.
Imaging and Labs
Members receive no cost and discounts (up to 95%) on lab diagnostics at over 4,000 patient draw centers across the country.
Imaging
Members will use discounted self-pay rates to pay for imaging. Costs associated with diagnostic imaging will apply towards your IUA for that medical Need or will be shared if your IUA has already been met for that medical Need.

Alternative Care
Some alternative medical treatments may be shared with prior written approval. For alternative medical treatments to be considered for approval:
The treatment plan must be considered safe and effective.
A member and licensed provider must be able to demonstrate the proposed value of the alternative treatment versus using current standards of care (also referred to as traditional or conventional treatment).
Approving alternative needs will be done on an equitable basis to the current standards of care of the same condition.
Alternative treatments that are more costly than the current standards of care may be capped on the amount that will be eligible for sharing. Choosing to start with alternative treatment and changing towards current standards of care may limit sharing based on what was shared for the alternative method
Toxic mold therapies, chronic lyme therapies, IV infusion therapies, ozone therapies, light therapies, and platelet rich plasma/stem cell (PRP) therapies are not eligible to be shared.
E.R./Urgent Care
Emergency Room
Emergency room visits are generally shareable separately or in conjunction with an eligible Medical Need related to an illness, injury, or accident. Members are responsible to meet their IUA for a Medical Need before sharing can begin.
Urgent Care
Self-pay. If this turns into a large medical Need, that out-of-pocket cost is applied to your IUA.
High Cost Medical
Your financial responsibility per medical condition (illness, injury, etc.) is capped by your chosen IUA amount. You only pay one IUA per medical condition or Medical Need.
You are responsible for no more than 3 in a 12 month period per membership. Choose between a $1000, $2500 or $5000 IUA.
Example: If you choose the $1,000 IUA and you break your leg and need surgery, medical expenses total $10,000. Your cost for this medical need is capped at $1,000 – including follow up care, provider visits and physical therapy, if necessary.
TIP: If you select the $5,000 IUA, you may be saving monthly, but if your family has 3 major medical needs in a year, you could be responsible for $15,000 ($5000 X 3) out of pocket.
Maternity
includes delivery, pre and post-natal care and postpartum depression treatment in 1 IUA
Maternity IUA differs from other shareable Needs in that they have a Maternity Initial
Unshareable Amount (Maternity IUA) of $5,000*. This applies to normal vaginal deliveries and emergency c-section deliveries.
Non-emergency/elective c-section deliveries have a total Maternity IUA of $7,500. Needs resulting from miscarriages will revert to the Member’s standard IUA.
*A pregnancy is eligible for sharing if it begins after 90 days of effective Membership and the estimated date of delivery falls one year or after the Membership becomes effective.
Pre-Membership
This membership limits sharing for premembership conditions.
The existence of a pre-membership condition does not prevent participation in the membership and is not intended to deny care. If you or someone in your membership has a premembership condition, sharing for that condition will not be shareable in the first year of membership.
Thereafter, sharing for premembership conditions that are otherwise eligible for sharing will increase based on the members tenure as follows:
- 2nd year: $25,000
- 3rd year: $50,000
- 4th year: $125,000*
A pre-membership medical condition is any illness or injury for which a person has been:
- Examined
- Diagnosed
- Taken medication
- Had symptoms or,
- Has a personal history of or known increased risk of conditions that may arise or worsen in pregnancy or
- Received medical treatment within the 24 months prior to their membership start date.
Exclusions: High blood pressure, high cholesterol, diabetes, are not considered pre-membership medical conditions as long as the member has not been hospitalized for the condition in the 24 months prior to joining, and the member is able to control the condition through medication or diet and exercise.

TOBACCO
Any Members that have used cigarettes, e-cigarettes, cigars, chewing tobacco, snuff, vape products, smoke cannabis, and pipe tobacco one or more times a month within the past year will be considered tobacco users.

An IUA, or initial unshareable amount, is your member responsibility for expenses related to a single eligible Medical Need. Designed to avoid costly surprise medical bills, IUAs function as caps on your out of pocket costs for eligible medical needs. You are responsible for a maximum of 3 IUA’s in a 12 month period. For many of our memberships, you have the freedom to select your IUA.
